This is a blank Illinois Environmental Protection Agency (IEPA) Clean Construction Demolition Debris (CCDD) form LPC-663 in editable PDF format. A3 Environmental Consultants wants you to have this in the event you have soil you need to dispose of. The CCDD facility is going to need your paperwork filled out and you need to get permission to dump the soil before you show up. You will also need a pH test done AND other soil sampling. In addition, you will need the signature and stamp of an engineer or professional geologist. The map below is an ArcGIS map of CCDD facilities and what’s called USFO or Uncontaminated Soil Fill Operations. This soil is presumed impacted by contaminants based on where the soil was gathered (on an or near a commercial or industrial property).
