This is a blank Illinois Environmental Protection Agency (IEPA) Clean Construction Demolition Debris (CCDD) form LPC-662 in editable PDF format. A3 Environmental Consultants wants you to have this in the event you have soil you need to dispose of. The CCDD facility is going to need your paperwork filled out and you need to get permission to dump the soil before you show up. You will also need a pH test done on the soil.
